Dartmouth Atlas of Medical Care Data

For more than 20 years, the Dartmouth Atlas Project has documented glaring variations in how medical resources are distributed and used in the United States. The project uses Medicare data to provide information and analysis about national, regional, and local markets, as well as hospitals and their affiliated physicians.

Unfortunately does not seem to be one consolidated dump so some work needed to list all resources.

No specific restrictions on data specified and made available for free download. Data is derived from Medicare material which is from US Federal Government (and hence likely public domain).
